GETTING STARTED To join the programme and collect rewards, complete and submit the course registration form below. Every time you book a qualifying course with Polson Higgs, you will earn loyalty points. Once you have reached a qualifying milestone, you can apply for reward vouchers. Conditions apply Examples of rewards Petrol Vouchers Beauty Product Vouchers Grocery Vouchers Movie Tickets Spa Treatment/Massage Vouchers Sporting Goods Vouchers Example Milestones 9 CPD points - $45 voucher (e.g 3 x 3 hour courses) 15 CPD points - $100 voucher (e.g 5 x 3 hour courses) 21 CPD points - $175 voucher (e.g 7 x 3 hour courses) 1 CPD hour = 1 CPD point

CONDITIONS OF REGISTRATIONS

• You must register to be eligible.
• Rewards points apply only to those courses stated as accruing CPD hours.
• Eligible courses for the Loyalty Programme commence 25th August 2011.
• Reward vouchers will be allocated only for those courses attended within the same 12 month period.
• Points are awarded to individuals.
• Points are not transferable.
• All vouchers to be redeemed with the supplier subject to suppliers usual terms & conditions.
• Polson Higgs has taken every care to provide vouchers for use only with reputable traders – but cannot be responsible for the item once uplifted from the supplier.
• Polson Higgs will contact you by email periodically to update you on your reward status.
• In the event of a cancelled course, reward points will not accrue.

FREE PRIZE DRAW

Every course booked automatically earns you a free entry into our mid-year prize draw to win a travel voucher, valued at $1500 from Brooker United Travel.

CONDITIONS OF PRIZE DRAW Annual mid-year draw takes places on the 30th June 2012 at Polson Higgs Dunedin Office at 139 Moray Place, Dunedin. The prize winner will be personally notified and announced on our website. One free entry for each CPD course purchased. The prize will be a voucher for travel arrangements and can be redeemed from the office of Brooker United Travel (subject to supplier's usual terms & conditions). The voucher must be redeemed by the 30th June 2013 and is not transferable or redeemable for cash and cannot be used to purchase foreign currency from Travelex.
